INFORMATION FOR WINDWARD STUDENTS REQUESTING LETTERS OF RECOMMENDATION
If I agree to write a letter of recommendation for you, please provide me with copies of any major essays or other projects you accomplished for our class. Let your college counselor know I've consented to write for you and fill out the Teacher Recommendation Form ASAP. Your responses should be as detailed as possible. You should confirm with your counselor that your name appears on my Naviance account.
INFORMATION FOR UCLA STUDENTS REQUESTING LETTERS OF RECOMMENDATION
If I agree to write a letter of recommendation for you, please provide the following items to help me craft a more effective letter:
Please give me at least THREE WEEKS advance notice so I can write your letter and mail it in a timely fashion. Please send me digital files (.doc., .docx., or .pdf) of your supporting materials. Please contact me at jeanpaul.deguzman <at> ucla.edu if you would like to discuss your letter further. If you need further assistance with your applications I strongly suggest you visit the following resources:
- Information on the graduate or professional program (or scholarship, award, job, etc.) you need the letter for, including the deadline for the letter;
- Any forms I, as the recommender, need to fill out or sign;
- If applicable, your "statement of purpose" or "personal statement" (whatever essay you will be sending along with your application);
- Your most recent resume or CV;
- Whatever research papers you wrote for our class, preferably with my comments;
- An addressed stamped envelope for my letter if I need to mail the letter separate from your application (my department does not provide postage). If I need to digitally upload the letter, please send me the link;
- Information you want me to emphasize (e.g., if you're having a professor write a letter focusing on your writing skills, an internship supervisor write focusing on your leadership skills, what, ideally, would you like me to focus on?). I will include that information at my own discretion.
